Effects of Povidone-Iodine and Running Load on the Synovial Membrane of the Knee joint in Rats


Abstract
The effect of povidone-iodine on the synovial membrane was studied in twelve week-old Sprague-Dawley male rats, weighing 250 g on the average. Eighty rats were equally divided into two groups for the experiment. 0.05 ml of an undiluted
povidone-iodine
in group¥°and 0.05 ml of 25% povidone-iodine in group¥±were injected twice into both knee joints with one week interval, respectively. Running load over the treadmill was also added until their sacrifice in two experimental groups. Ten were used
as
controls without any treatment. In each experimental group, 4 rats were sacrificed at 6th, 12th, 24th hour and 3rd day, and 6 rats at the end of 1st, 3rd, 6th, and 8th week after the second intra-articular injection, respectively. Both knee
specimens
including femoral condyles and upper ends of the tibia were taken. The pathological parameters for the histopathological studies of the synovial membrane were based on the degree of erosion, inflammatory reaction, fibroblastic proliferation, and
sclerosis.

@EN 1. Histological findings
1) The degree of synovial erosion at the 24th hour and at the end of the 6th week in group¥°was signiticantly higher in comparison with group¥±, respectively (P = 0.0256, p = 0.0027).
2) The amount of inflammatory infiltrate at the 24th hour and at the end of the first week in group¥°was significantly greater in comparison with group¥±, respectively (P =0.0287, P = 0.0079).
3) Fibroblastic proliferation of mild to moderate degrees in group¥°was shown at the end of the 1st week (75.0%) and the end of the 3rd week (58.3%). Fibroblastic proliferation in group¥±began to appera at the 3rd week (58.3%). Fibroblastic
proliferation in group¥±began to appear at the 3rd day, and proliferation of mild to moderate degree was shown at the end of the 1st week (66.7%) and the end of the rd week (50.0%).
4) Synovial sclerosis in group¥°was noted at the end of the 6th week (91.7%) and the end of the 8th week (91.7%). Synovial sclerosis of mild deree in group¥±was seen at the end of the 3rd week (41.7%), the end of the 6th week (50.0%), and the
end
of
the 8th week (58.3%).
2. Infuence of time and group variables on pathological parameters
1) It suggests that group¥°shows more erosive change of synovial membrane than group¥±.
2) The amout of inflammatory infiltrate in group¥°was greter in comparison with group¥±(P=0.0224). In due time it appeared to reduce the incidence of inflammatory reaction in both experimental groups with statistical significance (P=0.0138,
P=0.0000).
3) In due time it appeared to reduce the incidence of fibroblastic proliferation only in group¥±(P=0.0477).
4) In due time it seemed highly probable that synovial membrane was sclerotic in group¥°and¥±(P=0.0014, P=0.0007), respectively, and that group¥°showed more sclerotic change than group ¥±(P=0.0147).
These results suggest that povidone-iodine intraarticular injection with running load in rats can induce marked synovitis which then leads to diffuse synovial sclerosis, and that the injection of an undiluted povidone-iodine shows more sclerotic
change
that that of 25% povidone-iodine.
